Alerting signals, symptoms, evidence of occurrence: 
Donor autopsy (brain) showed Anaplastic T-cell lymphoma and transplant team was notified; Donor was originally suspected to have encephalitis clinically. No recipient symptoms but elevated aspartate aminotranferase (AST) on examination. MRI showed involvement of liver and multiple lymph nodes.
Demonstration of imputability or root cause: 
Multiple transmissions of T-cell lymphoma with FISH sex chromosome donor identification in one sex mismatched recipient.
