CNS data IPITTR (1994-2000): 33 donors with history of CNS tumor - 53 organs transplanted (25 from donors with astrocytoma, 21 glioblastoma, 7 medulloblastoma) - 9 (17%) transmissions: 1 astrocytoma III° (recipient dead), 5 glioblastoma (4 dead), 3 medulloblastoma (2 dead). CNS data UNOS (1992-2000): 418 donors with history of CNS tumor (no specific diagnosis described!) - 1129 organs transplanted - no transmission reported. Non-CNS tumors IPITTR: data from Ref. (239) Buell JF, et al. Ann Transplant 2004. Non-CNS tumors UNOS (4/1994-12/1998): From a total of 35.503 donors, 9 donors (0.025% donor transmission rate) WITHOUT evidence of malignancy transmitted a tumor to 12 recipients (4 melanoma, pancreas, lung, prostate, neuroendocrine, oncocytoma, leukemia, PTLD of donor origin; 0.01% organ transmission rate). Organ specific transmission rate: liver (n=7) 0.025%, kidney (n=3) 0.006%, heart (n=2) 0.013%. Of 488 donors WITH a history of cancer (skin and solid tumors, no specific diagnosis described) 1276 organs were transplanted without any reported transmission. Authors promote a rational use of organs from donors with malignancies to increase opportunities for transplantation but with appropriate respect for the risks.
