Alerting signals, symptoms, evidence of occurrence: 
Epithelioid granulomas with necrotic Schistosoma eggs found in the liver biopsy of a living donor who was not treated before donation. Recipient died of sepsis unrelated to schistosomiasis on day +14.
Demonstration of imputability or root cause: 
Schistosoma eggs found in liver granulomata without apparent disease transmission to recipient. After the acute phase, S. mansoni remains in the intestine; in the liver, necrotic eggs lead to granulomatous inflammation. Therefore, transmission of Schistosoma via the liver graft is unlikely to occur .
