Alerting signals, symptoms, evidence of occurrence: 
Using BacT/Alert, an apheresis unit was detected positive and was culture positive by anaerobic subculture but negative by aerobic culture. Gram stain showed Gram-negative S-shaped rods. The units were not released for transfusion and thus prevented a possible infection to recipients.
Demonstration of imputability or root cause: 
Campylobacter lari was identified in both platelet packs under anaerobic conditions on retest.
