8 and 9 months post-transplant, fever, weight loss, and pancytopenia. Postive blood and bone marrow cultures for Histoplasma capsulatum
Demonstration of imputability or root cause:
Pre-txp recipient serum samples showed no H. capsulatum. Donor lived in an area where histoplasmosis is highly endemic; kidneys appeared grossly normal at procurement. Recipients: The donor had a positive titer (1:16); a titer of 1:8 or greater was considered positive. The DNA-fingerprint patterns of the Histoplasma capsulatum isolate from the blood of Recipient 1 and blood and bone marrow of Recipient 2 were identical, according to RAPD-PCR analysis with the use of primers H1, H2, and H3. The DNA patterns of the isolates from Recipients 1 and 2, amplified with primer H1, differed from those of the Histoplasma capsulatum isolates from epidemiologically unrelated patients
