Alerting signals, symptoms, evidence of occurrence: 
Episodes of CMV reactivation at 6 - 60 days without prophylaxis. Serum antigenemia intermittently positive. One case had progressive lethargy, inattentiveness and disorientation. The other had seizures, fever and altered mental status. Both died of complications of CMV encephalitis.
Demonstration of imputability or root cause: 
Serum antibody, CMV viral load, CMV CSF viral load, CMV antigenemia. Magnetic resonance imaging showed ependymal contrast enhancement, non-enhancing periventricular white matter lesions and mild hydrocephalus . Electroencephalogram showed generalized slowing and periods of epileptiform activity. CMV PCR positive. Post-mortem examination of the brain of one of the cases showed CMV panencephalitis and ventriculitis with intranuclear and intracytoplasmic viral inclusion bodies
