Alerting signals, symptoms, evidence of occurrence:
Asymptomatic (~80%) in the majority of immunocompetent individuals;
Fever, malaise, myalgia, arthralgia, maculo-papular rash, diarrhea, meningitis, encephalitis, Neuroinvasive illess (WNND) in 40-60% of immunocompromised hosts.
Demonstration of imputability or root cause:
Donors had demonstrable markers of recent WNV infection at the time of organ recovery (retrospective testing). 85% of organ recipients (11/13) became infected within 2 weeks from transplant as demonstrated by Nucleic acid test (RTPCR), IgM and IgG in serum or CSF. The vast majority had clinical manifestations, mainly neurological.
