Alerting signals, symptoms, evidence of occurrence: 
Uncertain; liver recipient was traced back and identified after febrile kidney-pancreas recipient was found infected by T cruzi. The liver recipient, a 32 year old female, received several weeks of Nifurtimox therapy, but died of sepsis and multiorgan failure four months after transplant, cause of death thought to be unrelated to T cruzi.
Demonstration of imputability or root cause: 
Likely transmission. Although donor not tested, the donor was an immigrant from Central America. All three recipients of organs developed Chagas disease. All 3 recipient were peripheral smear and blood culture positive for T. cruzi
