Alerting signals, symptoms, evidence of occurrence: 
Blood eosinophillia two and a half months after liver transplantation as well as fever and hyperinfection with pneumonia. Unspecific duodenal ulcer histology was positive for parasites. There was a presence of parasites in sputum and stool.
Demonstration of imputability or root cause: 
Documented transmission in two recipients from the same donor. Both recipients were seronegative before transplantation and did not live in an endemic area. Donor was from Equador, with possible hyperinfection. Had received steroids before organ retrieval in the ICU. Results included positive serology and eosinophilia.
