Alerting signals, symptoms, evidence of occurrence: 
Endophthalmitis, Pain, Redness, Loss of vision. Patient normally presents with increased intraocular inflammation, hypopyon, vitritis 1 to 14 days after intraocular surgery. In this case: five months after penetrating keratoplasty, the patient developed two large endothelial plaques, an intrastromal abscess at the graft-host interface, and three large fluff balls in the anterior chamber. Cultures demonstrated Candida albicans.
Demonstration of imputability or root cause: 
Cultures of the intraocular fluid, aqueous humor or vitreous demonstrate causative organism. Possible transmission by PK: although there was no history of donor sepsis or long term respiratory therapy, the authors assume that the graft was the source of the infection.
