Alerting signals, symptoms, evidence of occurrence: 
A 46 year old female had sudden chills, dyspnea, and tachycardia; transfusion stopped and temperature increased from 97.9 F before transfusion to 99.5 12 hours after transfusion. Patient recovered after course of cefepime.
Demonstration of imputability or root cause: 
C. koseri isolated from patient and RBC unit, antimicrobial susceptibilities of both isolates matched, genotyping not done. After investigation, no source of the contamination was found.
