Alerting signals, symptoms, evidence of occurrence:
A pregnant 34 year old woman with β-thalassemia trait had nausea and fever to 39.3 C, progressing to leukopenia, thrombocytopenia, and elevated liver enzymes, and was diagnosed with transfusion-associated anaplasmosis at 32 weeks gestation, after receiving nine leukoreduced red blood cell transfusions. She was treated with rifampin, with good outcomes for mother and infant (child was not infected).
Demonstration of imputability or root cause:
Donor and recipient were positive for Anaplasma phagocytophilum by PCR (the donor was positive 4 days after donation).
