Man delivers transportion device with follicle fluid and sperm of partner to laboratory and pretends to be the partner. Because transport has taken too long the man is asked to produce a new semen sample. The treating physician is informed by the laboratory and informs the laboratory that the transporting man is not the partner
Demonstration of imputability or root cause:
Protocol requires male partner to deliver the transport box. Protocol is not followed. No photo-id check. Treatment is stopped
