Cryopreserved embryos of two couples were stored at the same date. When an embryo of one couple was thawed the straw had the name tag of the other couple. All embryos were not properly coded and named. 8 embryos were distroyed

Embryos of two couples probably mislabeled and then stored in liquid nitrogen, or placed in the wrong compartment with wrong color code
