Alerting signals, symptoms, evidence of occurrence:
Day before surgery two corneas were delivered to the hospital. Employee placed both corneas in the refrigerator. Detected on the next day. Both surgeries cancelled because of unsuitable corneas after storage at low temperature.
Demonstration of imputability or root cause:
Employee not familiar with corneal storage procedure. Corneas placed in the refrigerator.
