Chondrocytes came loose of the culture bottle during expansion. An incorrect culture medium was added (HBSS instead of DMEM)
Demonstration of imputability or root cause:
An incorrect culture medium was added (HBSS instead of DMEM). Patient must be subjected to a second cartilage biopsy.
