Alerting signals, symptoms, evidence of occurrence: 
Donor known positive, recipients under surveillance. Positive result for PCR, hemoculture and/or serology (TESA Immunoblot) at weeks 29 and 30.
Demonstration of imputability or root cause: 
Donor tested positive by serology and retested positive on IFA and ELISA. Recipient of bilateral lung tested positive while asymptomatic at weeks 29 and 30; Benznidazole started week 31 and completed. Reverted to PCR and later antibody negative but subsequently became positive again and provided with second course of Benznidazole. No report of outcome after week 43. Lung recipient never symptomatic. Of note the liver recipient never tested positive but had a complicated post-operative period ultimately ending in demise unrelated to T. cruzi.
