Alerting signals, symptoms, evidence of occurrence: 
A 63 year old female developed dysarthria, dysmetria, dysdiadokokinesis 15 months after corneal transplantation. She deteriorated with difficulty walking and swallowing, hyperreflexia, myoclonic jerks and paranoid hallucinations. She died 3 years 4 months after onset of symptoms. Autopsy showed findings of CJD. Organ donor data not provided.
Demonstration of imputability or root cause: 
Level 2. Possible transmission. Typical incubation period following cornea transplantation but because there is no donor data, this case could be a sporadic case, unrelated to transplantation.
