Lookback investigation of recipients of blood components donated by an HCV antibody-positive person in the 1970s and 1980s discovered a woman who was HCV-positive and had received a plasma transfusion in 1985 from the  HCV-positive donor. She under went a hip replacment in 1990 and donated her femoral head. In March 1990, it was used in another patient. Anti-HCV and elevated liver enzymes were found in the frozen femoral head recipient 22 weeks later in March 1990.
Demonstration of imputability or root cause: 
Level 4. Certain, Proven. HCV infection in blood recipient who then became a donor of a femoral head - the recipient was infected.
