Alerting signals, symptoms, evidence of occurrence:
Five days after lamellar keratoplasty a single suture abscess and small interfacial infiltrate developed. This was drained and cultured but no bacteria were grown. On postop day 7 the infiltrate worsened; an interface biopsy showed budding yeast and grew Rhodotorula sp. By day 20 the infection worsened and the donor button excised showing a normal button but the recipient bed showed Gram positive red yeast. The patient improved in one month. Donor eye culture showed no bacterial or fungal growth. No immediate pretransplant culture done.
Demonstration of imputability or root cause:
Unproven. Recipient infection but no evidance of donor tissue infection. No strong evidence of transmitted disease.
