Alerting signals, symptoms, evidence of occurrence: 
Fever developed in kidney-pancreas recipient 6 weeks post-transplant. The 37 year old female had T cruzi in blood and had a six month recurrent symptomatic infection despite two long courses of Nifurtimox. She died of Chagas myocarditis seven months after her original transplant.. The two other organ recipients were identified and proven to be infected with T cruzi. Liver recipient died of sepsis and multiorgan failure. Tje other kidney recipient was treated and improved. The organ donor was not tested for T cruzi.
Demonstration of imputability or root cause: 
Likely transmission. Although donor not tested, the donor was an immigrant from Central America. All three recipients of organs developed Chagas disease. All 3 recipient were peripheral smear and blood culture positive for T. cruzi
