Alerting signals, symptoms, evidence of occurrence: 
Uncertain; the 69 year old kidney recipient was traced back and identified after a febrile kidney-pancreas recipient was found infected by T cruzi. The kidney recipient received four months of Nifurtimox therapy and was well one year after her transplantation.
Demonstration of imputability or root cause: 
Likely transmission. Although donor not tested, the donor was an immigrant from Central America. All three recipients of organs developed Chagas disease. All 3 recipient were peripheral smear and blood culture positive for T. cruzi
