Two kidney recipients developed low grade fever 3 weeks postop, urine had white cells and grew A. fumigatus. Nephrectomy in both showed multiple A. fumigatus abscesses. All recipients recovered.

Level 4. Certain. Proven donor transmission of A. fumigatus since all recipients (2 kidney recipients, one heart recipient) developed A. fumigatus infection. Same fungus isolated from donor and recipient.
