Alerting signals, symptoms, evidence of occurrence:
A patient recieved islet cells for diabetes. Aspergillus fumigatus grew from the islets cells pretransplant. She was treated prophylactically for 3 months. She remained well without signs of infection.
Demonstration of imputability or root cause:
Level 2. Possible. Aspergillus fumigatus found in donor and in islets pretransplant although no clinical infection develped. Two kidney recipients developed A. fumigatus kidney infections.
