Alerting signals, symptoms, evidence of occurrence: 
At 12 yr, MDS presented and was treated with DLI. At 13 yrs, BM showed progression to RAEB (refractory anemia with excess blasts)
Demonstration of imputability or root cause: 
BM showed 80% positive for CD 14; cytochemistries also positive; 100% donor hemopoiesis.
Imputability grade: 
3 Definite/Certain/Proven
