Alerting signals, symptoms, evidence of occurrence: 
Recipient 1: urine culture positive for S.aureus on day 5 after transplantation; sepsis syndrome and bleeding from arterial anastomosis on day 7. Recipient 2: fever on day 2; bleeding from arterial anastomosis of day 9. Both recipients underwent transplantectomy. One recipient died after transplantectomy due to bleeding from the external iliac artery.
Demonstration of imputability or root cause: 
Donor with severe burns. No blood cultures were performed. Urine culture and swabs from burns were negative. Both recipients developed sepsis with the same S.aureus strain according to phage typification using blood and urine cultures.
