An important observation is the fact that the donor did not have leukemia or show evidence of the translocation and remains well, and the recipient had an earlier posttransplant karyotype that was normal. This leads to the conclusion that, although the leukemia is arising from donor cells, it is most likely a post-transplant event. In the terminology of solid organ transplantation, it can be considered a "donor derived" but not "donor transmitted" cancer.
