This systematic review (updated to October 2019) documents 14 studies that describe 16 patients who received kidneys from donors with angiomyolipoma. 65% (n=9) of patients had ex vivo resection, 18% (n=3) no resection, and 17% in vivo resection (n=2) or not reported (n=1).
In those who did not have resection, the lesion size remained stable and none had malignant transformation. No donors with angiomyolipoma developed end stage renal disease.
It is important to note that none of the donors had angiomyolipoma associated with tuberous sclerosis complex or with lymphangiomyoleiomatosis (in 9 patients the tuberous sclerosis complex status was not reported).
The authors concluded that their findings support the safety of these kidneys for transplant. However, we also note the small numbers of reported cases.
