Alerting signals, symptoms, evidence of occurrence: 
Fever, cramps in the right leg and sudden left loin pain (subsequently diagnosed as embolism to the left leg).
Demonstration of imputability or root cause: 
The allograft was excised because of severe aortic insufficiency. At operation, thrombotic vegetation attached to the graft's free margin, Paecilomyces varioti was cultured from the remnants at autopsy and from tissue that was removed surgically. The long period before clinical symptoms seem to suggest post transplant acquired infection, nevertheless the authors don't exclude that the infection may have been introduced with the allograft.
