A 42 year old woman underwent bilateral living-donor (an elder brother and a younger sister) lobar lung transplantation. Before transplant the younger sister was positive for anti-HTLV-I antibody, but there was an urgent need for a suitable organ. The lung transplantation was successful however, serologic follow-up demonstrated that the patient had acquired HTLV-I infection posttransplantation. HTLV-I antibodies were first detected at 9 months, with subsequent increase in titres. Approximately 5 years posttransplant, the patient demonstrates neither neurologic nor hematologic evidence of HTLV-I related disease.
Demonstration of imputability or root cause:
Serologic follow-up demonstrated that the patient had acquired HTLV-I infection posttransplantation. One of the donors was known to be HTLV I seropositive before donation. No molecular studies were performed.
