Of the 8 patients with presumed CMV transmission from islet transplantation, seven patients had asymptomatic seroconversion, one patient presented flu-like symptoms during H1N1 epidemic and treated with Valganciclovir and Oseltamivir 193 days after islet infusion.
Demonstration of imputability or root cause: 
A total of 121 type 1 diabetes mellitus patients underwent 266 islet infusions over a period of more than 11 years at a single institution. Analyzing by transplant procedure (n = 266) the CMV status of the D/R pair was: D–/R–: 29.3% (n = 78); D+/R–: 31.2% (n = 83); D–/R+: 13.2% (n = 35); and D+/R+: 26.3% (n = 70). CMV prophylaxis was used in 181 of 266 patients (68%, D+/R– procedures, or R+ patients who received antilymphocyte globulin induction). Overall CMV infection occurred in 14/121 patients (11.6%). Presumed transmission from islet transplantation occurred in 8/83 of CMV D+/R– procedures (9.6%): 6 seropositive, 1 viremia, and 1 with CMV disease.
