Alerting signals, symptoms, evidence of occurrence: 
Patient received one unit of red blood cells (RBCs), and ten days later was readmitted in painful crisis with Hb of 4.2 g per dL. Patient was given phenotypically matched group O RBCs immediately, but developed hemolytic transfusion reaction (HTR) symptoms of chills, fever, tachycardia, and laboratory signs of hemoglobinemia, hemoglobinuria, and negative DATs.
Demonstration of imputability or root cause: 
Cold reactive autoantibody transfusion with blood warmer resulted in no adverse event.
