Alerting signals, symptoms, evidence of occurrence: 
Patient with history of documented anti-Ge3 alloantibody who received multiple units of Ge+ red blood cells (RBCs) came in to receive 8 units of Ge+ RBCs with no incident. 4.5 years later the patient received 2 units of Ge+ RBCs and developed clinical and laboratory signs consistent with AHTR.

Serology and no increase in hemoglobin concentration
