Shock resistant ventricular fibrillation and flabby heart, hypotension. During aortic clamp release during cardiac surgery. The
transfused blood (unwarmed) was found to be 16 days old with a potassium concentration of 16.6 mmol.l^1.

Following massive blood transfusion of stored blood, complications such as hyperkalaemia and citrate toxicity are well recognised, but fortunately very rare.
