The loss of sperm procured by an invasive procedure (PESA). Sperm was transported in cryocontainer to another clinic to be used next day for ICSI. On arrival, the sperm was no longer viable. The cryocontainer that was found at retrospective validation to maintain the required conditions for less than the 24 hours stated in the specification.
