Alerting signals, symptoms, evidence of occurrence: 
Patient received 2 CBU units. One was RBC replete. A volume of 126 mls of lysed RBC was infused. Transfusion of lysed red cells, nitric oxide scavenging by free Hb, vasospasm/smooth muscle spasm w severe, widespread pain. Gross haemoglobinuria. Able to be managed with aggressive fluid resuscitation with preservation of renal function and without long term sequelae.
