Retrospective review: TACO occurred 1 per 1566 transfused plasma units. Active surveillance: 1 per 68 plasma units transfused (none reported to the transfusion service). Patient comorbidies included renal failure, congestive heart failure, or respiratory failure. The TACO patients received an average of 4.0 plasma units at an average dose of 9.5 mL/Kg, and average rate of 647 mL/hour.  .

Alerting signals, symptoms, evidence of occurrence: 
Dyspnea, increased oxygen requirements, elevated jugular venous pressure, hypertension, tachycardia, tachypnea, crackles or rales on lung ascultation, response to diuretics, increase in BNP over baseline, CXR evidence of new onset or worsening congestive heart failure.
Demonstration of imputability or root cause: 
Expert review of electonic medical records for retrospective review and expert case review during active surveillance period.
