H/O CHF (41%), renal dysfunction (44%), age > 70 years (56%) were commonly observed factors in TACO cases. Median volume blood ordered = 500 mL; median preceding 24 hour crystaloid/colloid = 2200 mL. 18% of TACO patients to ICU, 8% had major complications, and 2% died.

Alerting signals, symptoms, evidence of occurrence: 
100 consecutive cases at two academic centers. The patients developed dyspnea, cyanosis, orthopnea, hypertension, or congestive heart failure following a transfusion.
Demonstration of imputability or root cause: 
All cases reviewed by transfusion safety nurse and transfusion medicine physician.
