Estimated frequency: 
14.4% of platelet transfusion episodes associated with acute transfusion reaction.  Of these, 45.0% were transfusion associated dyspnea (acute respiratory distress within 24 hours of trasfusion cessation and all other causes or reaction ruled-out) and 4.2% TACO.  No TRALI events. 10 of the 16 with TAD required respiratory support. No deaths.

One year retrospective review of electonic medical records of all patients <18 years who received platelet transfusions. Inclusion criteria: reportable acute transfusion reactions per US Hemovigilance definition.
Demonstration of imputability or root cause: 
For the authors assessment the TAD and TACO case imputability--probable.No CVC or B natriuretic peptide testing performed to differentiate TACO or TRALI from TAD.
