Alerting signals, symptoms, evidence of occurrence: 
Patient with diabetes mellitus, chronic renal failure, chronic heart disease and other co-mobidities, received a red blood cell transfusion following a coronary artery by-pass. On day +2 he developed fever, malaise and hepatitis, worsening of thrombocytopenia and worsening of pleural effusion. DENV-2 detected in blood by PCR. Treated supportively, recovered. Seroconverted to dengue IgG and IgM.
Demonstration of imputability or root cause: 
Asymptomatic repeat donor at time of donation, reported fever and myalgia 1 day post donation. DENV-2 detected in blood by PCR. Three recipients of blood components (RBC, platelets, FFP) from same donation were diagnosed with DENV- 2 infection within 2 days from transfusions.
