Alerting signals, symptoms, evidence of occurrence: 
58 year old man with idiopathic medullary aplasia received 3 erythrocyte units. While receiving 3rd unit, patient became febrile to 40 °C and had chills and headache. Septic transfusion reaction suspected and gram stain of blood smear from 3rd unit showed large number of gram-variable coccobacilli. Patient placed on broad spectrum anticbiotics, followed by augmentin and ofloxacin, resulting in "rapid recovery".
Demonstration of imputability or root cause: 
Psychrobacter grew from unit and from patient blood culture; DNA sequences identical.
