Alerting signals, symptoms, evidence of occurrence:
Recipient 1: fever, abdominal pain, and drainage from the surgical incision. Blood cultures obtained when transplant team was notified of positive donor culture results, were positive for Pseudomonas aeruginosa on day +4. The P. aeruginosa isolated from blood was found to be MDR, sensitive only to polymyxin B and amikacin, and the patient was started on polymixin B. Recipient 2 died from pseudomonal infection.
Demonstration of imputability or root cause:
Recipient’s isolate from blood and the donor isolate from the lungs were found to be indistinguishable by pulse-field gel electrophoresis, confirming the diagnosis of donor-derived infection due to MDR P. aeruginosa.
