A 86 year old man with upper gastrointestinal bleeding, transfused red blood cell (RBC) unit with onset of chills, dyspnea, tachycardia, temperature to 39.6 °C. Recovered after antibiotic therapy and was discharged 3 days later.
Demonstration of imputability or root cause:
Organism isolated from patient peripheral blood culture and from RBC package; isolates matched by antimicrobial susceptibility and PFGE (pulsed field gel electrophoresis).
