Alerting signals, symptoms, evidence of occurrence:
Fever, multiple blood cultures positive for E. Coli, rise in AST to 2000 U/L and poor flow in the portal vein noted on Doppler ultrasound.
Demonstration of imputability or root cause:
Donor blood cultures were negative at the time of organ procurement, but became positive after 48 hours of incubation. PFGE (pulsed field gel electrophoresis) confirmed the donor and recipient strains were indistinguishable. No evidence of postoperative infection in the renal recipient from the same donor.
