Alerting signals, symptoms, evidence of occurrence: 
Recipient 1 (R1): sore throat and high fever with generalized malaise and rigors. Recipient 2 (R2): fever, wheezing and dysuria. Recipient 3 (R3): asymptomatic, his AdV infection was detected because of protocol renal biopsies for resolving delayed graft function.
Demonstration of imputability or root cause: 
Three recipients received kidneys from 2 deceased donors (D1 to R1 and R2, D2 to R3). AdV testing in the donors not available. Negative pre-transplant serology for the recipients. Immunohistochemistry, BAL, urine and nephrectomy specimen positive for AdV DNA. The relatively early onset of the AdV infections in the recipients R1 and R2 from the same donor (D1), and the recipients’ ages of 44 and 56 years, argue against both recipients developing spontaneous AdV infections independent of the donor graft. The 4-year-old donor (D2) appears to have transmitted AdV 2 and 6 to R3. Both serotypes are common in the pediatric age group.
